Tl'azt'en Nation is seeking a dynamic individual to engage our community in positive health and wellness activities through best practices and clinical health indicators that reflect the social determinates of health. The successful candidate will be a key member of our clinical health team reporting to the RN. The ideal candidate is motivated, a self-starter, strong planner, community minded, team oriented ready to step up and provide excellence in service to our community. In accordance with the Mission, Vision and Values, and strategic directions of the Nation and the directive of Community Health Services (CHS), patient safety is a priority and a responsibility shared by everyone at CHS, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Duties and Responsibilities
- Works collaboratively with the interprofessional team to provide health care services to clients with physical and/or mental health, substance dependence, concurrent disorder and chronic disease. The LPN identifies and monitors clients actual and potential strengths and limitations including psychosocial, physiological, psychological, social-cultural and spiritual needs. - Duties also include planning, implementing and evaluating nursing care within an interprofessional clinical framework in accordance with Standards of Practice for the Licensed Practical Nurses of British Columbia. - Adheres to all Nation policies, procedures and is an ambassador for the Community Health Services Team. -Work Schedule Tuesday-Saturday with the expectation of evening work.
- Graduation from a recognized program for Practical Nurses plus 1 year of experience working with offenders and/or in secure health care settings, or an equivalent combination of education, training and experience. - Current full practicing licensure with the College of Licensed Practical Nurses of B.C. (CLPNBC). - 1-3 Years of relevant work experience (LPN) Valid Class 5 Driver's Licence, Class 4 Preferred - Subject to a Criminal Record Check & Vulnerable Sector Check prior to employment - Previous experience working in First Nation Community - Experienced with MS Office, Internet and general computer proficiency -Valid First Aid and CPR - Any other relevant certifications
